The Admission Services Manager (ASM) is responsible for the
direction and management of Intake & Admission functions and
Professional Outreach of Hospice of Santa Cruz County. ASM directs
the daily operations of the intake and admission and outreach
process to ensure timely and appropriate access to Hospice and
Transitions services.
The ASM is a member of the Nurse Management team responsible for the
direction and management of clinical services across all functions.
The ASM demonstrates: clinical expertise in assessment, pain and
symptom control, understanding of prognosis criteria and hospice
philosophical alignment; experience in outcome based relational
marketing with sensitivity and compassion for the issues involved in
serving patients and families facing a terminal illness, death and
bereavement; expertise in issues related to 3rd party reimbursement;
management and leadership skills and knowledge; program development
skills. Responsible for ensuring compliance with the Medicare
Conditions of Participation (CoPs) and all other regulatory
requirements, with respect to appropriateness of referral,
eligibility for services and required documentation.
Essential Responsibilities:
ADMISSION SERVICES MANAGEMENT
Based on agency goals, sets overall direction for
professional outreach, intake & admission functions.
Directs daily operations of professional outreach, intake &
admission process ensuring regulatory compliance
INTAKE & ADMISSIONS
Develops/ensures customer service focus for intake and
admissions process, ensuring timely and appropriate access to
Hospice and Transitions.
Ensures eligibility & regulatory compliance of admission
process.
Meets agency goals and budget targets for department or
agency growth
Utilizes industry expertise in leading team to achieve agency
goals re: conversion rate, referral to admit standards, etc
Develops systems for & reports referral data, response time, etc
Ensures accurate and timely reporting.
Promotes and trains to agency Open Access policies.
Evaluates and improves intake and admission process to
increase responsiveness and improve productivity.
Manages referral flow & response, priorities, assignments.
Ensures appropriate funding source. Monitors for accuracy and
to identify trends.
Assigns staff to ensure appropriate utilization in meeting
departmental, interdepartmental & agency goals.
Makes recommendations related to admissions, census and
resources.
Monitors Hot Spots, report F/U, tracks for trends.
Directs systematic check & feedback on each admission.
Works collaboratively to ensure easy flow of utilization of
professional staff between departments.
PROFESSIONAL COMMUNITY ENGAGEMENT
Responsible for preparation and implementation of annual
outcome based, relational marketing plan that strengthens existing
referral base and identifies viable new opportunities for
appropriate
Hospice and Transitions referrals, resulting in reaching targets
set for:
Skilled nursing facilities.
Residential care facilities for the elderly.
Home
Directs marketing activities of Transitions Program Manager,
Hospital Liaison and Facility Liaison to ensure outcome oriented
engagement with
Hospital Discharge Planners
SNF (Skilled Nursing Facility) and RCFE (Residential Care
Facility for the Elderly)
Physicians
Home care agencies
Other referral sources
Works collaboratively with Development and Community Outreach
to ensure consistent and target messaging.
Staff Management and Supervision
Utilizes practices of Rounding, Thank Yous and Key Words at
Key Times to ensure staff engagement and agency responsiveness.
Manages, Assigns, Evaluates all assigned staff.
Participates and coordinates in orientation and training of
assigned staff.
Ensures appropriate and compliant documentation per HSCC
standards for assigned staff.
Ensures/develops staff accountability to identified outcomes.
Insures job descriptions are kept current and utilizes them
to help define employees duties and responsibilities.
Maintains clinical expertise to lead teams to optimal
clinical outcomes.
Provides continuous and timely feedback, support and
mentoring to assigned staff. Directs and corrects performance of
staff when appropriate.
Provides timely and skillful feedback related to employee
performance. Prepares timely annual evaluations for assigned staff
Develops outcomes, standards and program improvements for assigned
staff.
Review evaluations with supervisor and HR Director for input
and support.
Advises supervisor of significant disagreements about
performance.
Sets annual goals in alignment with agency goals for assigned
staff.
Develops corrective plans of actions when needed.
Approves time logs ensuring verification of time worked, paid
time off and review of activities for reasonableness.
Manages vacation planning/approval/coverage process for
assigned staff.
Reviews monthly reports of available sick and vacation time
and discusses with staff, including advising of caps for sick and
vacation.
Coordinates any requests for leave of absence with H.R. by
requesting the LOA Form and ensuring completion and return. HR
Director will advise and coach necessary steps and communication
between supervisor and employee as needed.
In case of an on the job injury, complete supervisors
section of the workers compensation claim form and send
immediately to Controller. Controller will advise and coach
necessary steps and communication between supervisor and employee
as needed.
EDUCATION/EXPERIENCE 5 years previous Healthcare
Supervisory/Management experience strongly preferred with Hospice
management experience desired. RN preferred. Experience with
admission flow is helpful.
SKILLS REQUIRED: Critical thinking, good judgment, leadership, team
building, inspires confidence and a strong customer service ethic,
change friendly/adaptable, strong verbal and written communication
skills, ability to bridge regulations with clinical practice,
ability to see larger picture yet capable of netting out details and
action plans, solid at developing and maintaining working
relationships, exhibits flexibility, and discernment in decision
making. Can think on their feet and verbalize well in response to
challenges. Flexible leadership style that drives to getting the
best from staff. Ability to stand firm and remain committed when
necessary and facilitate understanding when difficult or adverse
decisions must be addressed or made, End of life clinical skills
preferred.
CERTIFICATIONS, LICENSES, REGISTRATIONS:
Active State of Ca RN license preferred, current drivers license
required Hospice and Palliative Certification preferred.
